Discussion was held on the topics of current enrollment, current staffing, and a review of changes to background checks with regards to expiration dates, new fingerprint checks to be done through Fieldprint of Virginia & Office of Background Investigations (OBI), new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years. The roll out for the new fingerprint checks will occur on January 16, 2018. More information shall be sent to you about this process. All finger print checks for existing employees must be started for each according to the established time table and then completed by September 30, 2018. New employees will not be allowed to start until an ?Eligibility Letter? is on file. Discussion that the Immunization page on file for Child #1 appears to be incomplete. Please, talk to the child's parent to obtain a page with a record that reflects all shots administered.
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Staff of Villa Montessori Preschool located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 50 children with 8 classroom staff; good ratios in all 4 rooms. A total of 5 children?s files and 5 staff files were reviewed. The children were engaged in the following: large motor skilled activities on the center playground, bathroom breaks, hand washing, diaper changing, games, song, circle time, story time, centers within the classroom, lunch, learning, nap time, music, and other organized activities. Standard #: 22VAC40-185-140-A Description: Based on review, 2 of 5 children's files reviewed indicated that each child did not have a physical examination (by or under the direction of a physician) within one month after attendance. Evidence - 1. On the date of inspection (01/11/2018), documentation of a physical examination was not on file for the following children: Child #1, and Child #2. 2. The start date for Child #1 was listed as - 09/13/2017. The start date for Child #2 was listed as - 09/25/2017. Plan of Correction: A parent for each applicable child shall be contacted and the missing information shall be obtained and filed.
