22VAC40-191 Background Checks (22VAC40-191)
The licensing inspector conducted an unannounced renewal inspection today during a non- public school day. There were no children in care as they were in care at another licensed facility. The facility inside and outside was reviewed along with staff and children files and other documentation. There are 27 children enrolled aged 5years-12years; and 2 staff and the director. Thank you for your cooperation. This inspection began at 9:30am and concluded at 11:15am.
Standard #: 22VAC40-185-60-A Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to parental information and custodial documentation. Evidence: Child records #1 and #2, start date 12/21/2017, did not contain the biological fathers information as required. Also the father was listed as not being able to pick up the children, but no supporting custodial documentation was available. Plan of Correction: The mother will be asked to supply the missing information upon pick up today. If no custodial information is available then the father will be removed from the non-pick up list until documentation can be provided.
