A Subsidy Health and Safety inspection was conducted on 1/11/18 to review supplemental health and safety requirements. An unannounced renewal inspection from the provisional license was made on 1/11/18. There were 42 children present from four years through 11 years old with five staff supervising and the child care director present in addition. Six children's records and six staff records were reviewed. The inspector arrived at the center at 2:00 pm and departed at 5:15 pm. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. The staff were reminded about completing and documenting a lock down drill annually.
Standard #: VENDSUB-000-006-B-11 Description: Based on record review and interview with staff, the center failed to ensure that there was a written allergy care plan for each child with a diagnosed food allergy. Evidence: 1. Child #6 had a physical examination completed 9/1/16 and the physical form was signed by the physician on 8/2/17. There was documentation on the physical examination made by the physician that the child was allergic to milk. There was no allergy action plan for the child at the center. Plan of Correction: The site director will talk with the parent to get clarification of the food allergy. Based on that conversation, the center will get proper documentation from the physician and parent based on the directions for the food allergy.
