Comments:
Today's visit was a routine monitoring inspection. The inspection began at 9:30 and concluded at 3:00pm. There were sixty-four children present and twelve staff members. School age children were present due a local school observed day off. During the inspection a total of forty minutes of classroom observations took place in all seven classrooms, infant care was observed, lunch time was observed, the center's emergency plan was viewed, six out of ninety-one children's records were reviewed, six out of seventeen staff records were reviewed, a sample of injury reports were reviewed, and some additional policies and procedures were reviewed. Discussion took place regarding the requirement to report serious injuries and background check requirements. Discussion also took place regarding the playground structure that was recently placed over concrete and is currently reported to be off limits to children due to lack of resilient surfacing. Standard #: 22VAC40-185-130-B
Description: Based on the inspector's review of six out of ninety-one children's records, the center failed to ensure immunization records were updated once every six months for children under the age of two. Evidence: Child #5 has an immunization record dated 9/2/15 when the child was three months of age. There are no other immunization records in the child's file.

Plan of Correction: The PD will contact the parent for a copy of the child's most recent immunization record.

Standard #: 22VAC40-185-450-C
Description: Based on the inspector's interview in an infant classroom, the center failed to ensure the linens in cribs on a daily basis. Evidence: The inspector asked the infant room teacher how often sheets for the cribs are changed and she replied "weekly". Crib sheets are required to be washed daily.

Plan of Correction: The PD will remind all staff crib linens are to be washed daily.

Standard #: 22VAC40-185-550-D
Description: Based on the inspector's review of logs for emergency drills, the center failed to ensure emergency evacuation drills were practiced and recorded for August and October 2017. Evidence: The center failed to practice and record monthly emergency evacuation drills for two months.

Plan of Correction: The PD will ensure monthly fire drills are conducted and documented.

Standard #: 22VAC40-185-560-F
Description: Based on the inspector's observations and menu review, the center failed to follow current USDA child meal patterns. Evidence: Lunch did not include a vegetable component.

Plan of Correction: The PD will review USDA requirements and update the menu.
