An unannounced renewal inspection was conducted today, January 19, 2018 to determine compliance with licensing standards. The inspector arrived at 12:00 pm and concluded the inspection at 3:00 pm. Upon the inspector arrival children were having lunch in their classrooms. Children provide their own meals which were labeled with names and today's date. After lunch children had time playing with puzzles, and looking at books before preparing for nap time. Bathroom breaks and hand washing was conducted. Staff in the three year old classroom read two stories to the children before putting soft music on for nap. All areas of the facility to include classrooms, bathrooms, hallways, and playground area were inspected. Classrooms have age and stage appropriate materials and supplies. Children's winter artwork is displayed. Children in the four year old classroom had constructed a snowman out of boxes, painted it white and decorated it with a scarf and buttons to make a face. The facility had the license, last inspection notice, emergency numbers, emergency evacuation procedures/maps (See violation notice), daily schedule and various parent information posted. The first aid kit and the non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Five staff records and five children's records were reviewed. Staff on site have current CPR, First Aid and daily health observation training. Multiple staff has current MAT certification. Medications and authorizations were reviewed. The most recent evacuation drill was on 12/22/2017. The two required shelter-in-place drills for 2017 were completed on 01/31/2017 and 11/31/2017. A lock down drill was documented on 01/31/2017. Injury reports were reviewed. Today's Ratio's: Classroom #1: (4-5 years) 8 children with 1 staff. Classroom #2: (3 years) 3 children with 1 staff. Classroom #3: (2 years) 3 children with 1 staff. Classroom #4: (Toddler) 2 children with 1 staff. Fire Inspection: 06/21/2017 Health 01/30/2017 The violations from the previous inspection on July 24, 2017 were checked for corrections. There were no repeat violations cited during today's inspection. A complete renewal application will need to be submitted and received by the Department before the license expiration date. Standard #: 22VAC40-185-290-3 Description: Based on observation, the facility did not ensure all electrical outlets had protective covers installed. Evidence: 1. An electrical outlet next to the bookcase in the three year old classroom did not have a protective cover installed. 2. An electrical outlet in the Toddler classroom did not have a protective cover installed. Plan of Correction: Staff installed the protective cover per the inspectors request.
Standard #: 22VAC40-185-550-B Description: Based on review of emergency preparedness procedures, the facility did not ensure the emergency preparedness plan contained all required procedural components. Evidence: 1. The procedures for emergency communications did not include the establishment of center emergency officer and backup officer to include 24-hour contact telephone number for each; 2. The procedures for evacuation did not include secondary means of egress and methods of communication after the evacuation; 3. The procedures for shelter-in-place did not contain method of communication after the shelter-in-place; 4. The emergency preparedness plan did not contain procedures for facility containment, staff training requirements, drill frequency and the plan review and update. Plan of Correction: Per the Owner: I could not locate my original emergency preparedness plan. I will create a new document to include all required elements and have it reviewed by local authorities.
