Comments:
An unannounced investigation triggered by a complaint received on 12/21/17, was conducted today between 1:25pm to 2:00pm. The complaint was related to a child that received breakfast items on 12/20/17 by staff, that the child was not permitted to eat due to allergies. Based on the preponderance of evidence, the complaint was valid. Another non-complaint related violation was noted during this investigation. Description: Based on record review, the center did not obtain the proof of child identity. Evidence: 1. Child # 1's date of enrollment is 12/5/17. 2. There was no documentation to show that the Child # 1's proof of identity was reviewed by the center.

Plan of Correction: We will get the proof of identity from the parent.

Complaint related: Yes
Description: Based on staff interview and records review, staff did not ensure care, protection and guidance for children while supervising them. Evidence: 1. On 12/20/17, Child # 1 was served french toast sticks with pancake syrup for breakfast. One of the ingredients for the pancake syrup includes corn syrup. Child # 1 is allergic to corn and any food items that may have any corn, such as corn syrup. This information was listed in the child's registration form at the time of enrollment dated 12/5/17, and was reviewed by the licensing inspector during the investigation. 2. The morning of 12/20/17, Staff # 2 was helping serve breakfast to the group of children that included Child # 1. Staff # 2 served Child # 1 french toast sticks with pancake syrup. Staff # 2 did not check the allergy list before serving breakfast. Child # 1 informed his parent at pick up time, that he ate french toast sticks with syrup for breakfast. Child # 1's parent spoke to Staff # 1 about this and expressed concerns about why the child was being served food items that he is not permitted to eat. 3. Staff # 1 also stated that when the parent approached her about the issue, she told the parent that an allergy list is posted in each classroom, and Staff # 2 probably did not check the allergy list before serving breakfast. 3. Staff # 1 showed the allergy list to the licensing inspector during the investigation. Upon questioning, Staff # 1 confirmed that Child # 1 was served the breakfast items by Staff #2. 4. As a result of eating the breakfast items, the child developed a fever, which is his reaction to eating any food items with corn or corn syrup. 5. Child # 1's parent also contacted the owner of the center to discuss this issue. 6. Staff #1 stated a memo was sent out to all center staff re-training them about the appropriate measures that must be take to ensure that children with allergies are not served any food items that they not permitted to consume. This memo was available during the investigation and was reviewed by the licensing inspector. The memo was signed by all staff members. 8. Staff # 2's hire date is 9/18/17. Staff # 2 has received orientation training and First Aid /CPR training.
