Thank you for your time and assistance with the unannounced monitoring inspection conducted today, January 22, 2018. The on-site inspection was conducted from 11:20 a.m. to 5:45 p.m. At the time of arrival there were 66 children in six classrooms and in the care of 12 staff members with two staff members available to assist if needed. The total enrollment for the program was 133 children. The records for seven staff members were reviewed, and seven children's records were reviewed. There were eight staff members and four children interviewed. The medication and medication paperwork were reviewed for two children. The exceptions to compliance were noted on the violation notice. Lunch was observed to meet the USDA guidelines and consisted of chicken, macaroni and cheese, broccoli, peaches, and milk. The theme of the week was Winter Sports. Some items observed and reviewed were: injury reports, required posted information, emergency and shelter-in-place drills, water temperature, story time, infant care, infant feeding, diapering procedures, hand washing and bathroom procedures, outdoor play, lunch, nap time, and dismissal procedures. Standard #: 22VAC40-185-130-B
Description: Based on review of seven children's records and discussion with the director, the center failed to obtain documentation of additional immunizations for one child under two years of age. Evidence: 1. The record for Child #6 did not contain documentation that the child had received additional immunizations every six months since the date of enrollment. The only immunization record was dated 10/31/2017 and the child's date of enrollment was 11/11/2017. 2. The director confirmed that there were no additional immunizations in the child's record.

Plan of Correction: During the inspection, the center obtained the updated immunization record for the child.

Standard #: 22VAC40-185-140-A
Description: Based on review of seven children's records and confirmation from the director, the center failed to obtain documentation that one child had received a physical examination. Evidence: 1. The record for Child #4 did not contain documentation that the child had received a physical examination by a physician. Date of enrollment for the child was 12/11/2017. 2. The director confirmed the child's record did not contain a physical examination report.

Plan of Correction: During the inspection, the center obtained a copy of the child's most recent physical examination.

Standard #: 22VAC40-185-450-A
Description: Based on observation and discussion with staff, the center failed to provide a bottom linen for two children. Evidence: 1. In Classroom C, two children were napping on their cots with only a top cover. There was no bottom linen provided for the children. 2. Staff Member A stated that the parents did not provide linens for the children and the one linen that each child was using was a cover from the center.

Plan of Correction: During the inspection, two bottom linens were provided for both children.
