Comments:
A Subsidy Health and Safety inspection was conducted on January 22, 2018 to review supplemental health and safety requirements. The on-site inspection was from 11:20 a.m. to 5:45 p.m. On this date, there were 66 children present and in the care of 12 staff members. There were two additional staff members available to assist if needed. There were 133 children enrolled in the center. Seven staff members' records and seven children's records were reviewed and six classrooms were observed. Eight staff members and four children were interviewed. The medication and medication paperwork were reviewed for two children. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on the review of six records of children with diagnosed food allergies, the center failed to obtain written allergy care plans for four children. Evidence: 1. The records for Child CC, Child DD, Child EE, and Child FF did not contain written allergy care plans. The children had all been diagnosed with a food allergy. 2. The director confirmed that the center had not obtained allergy care plans that included instructions from the physician for the four children.

Plan of Correction: The director will contact the parents of the children immediately and obtain an allergy care plan that has instructions on what steps need to be taken in the event of a suspected or confirmed allergic reaction.
