An unannounced monitoring inspection was conducted during the aftercare portion of the program on January 19, 2018. There were two staff present with 15 school age children. Children were observed having snack which consisted of cereal and milk, playing freely with tabletop activities, and talking freely amongst themselves. Students also visited the Book Mobile. Five children's records were reviewed on this date. Five medications were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, hand washing, nutrition, injury report documentation, medication procedures, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, indoor areas, emergency supplies and required postings. Violations were reviewed with and verified by the Recreation Leader.
Standard #: 22VAC40-185-240-D-5 Description: Based on review, the licensee did not ensure that there shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Evidence: During the inspection conducted on January 19, 2018, there were two staff members present with no documentation of current DHO training. Plan of Correction: Waiting for the production of the staff member's certificate.
Standard #: 22VAC40-185-510-C Description: Based on review, the licensee did not ensure that the center's procedures for administering medication shall include methods to prevent use of outdated medication. Evidence: During the inspection conducted on January 19, 2018 the licensing inspector observed an expired emergency medication in the medication box. The inspector requested to review the policy regarding preventing the use of outdated medication. Staff members verified that the medication policy did not address outdated medication. Plan of Correction: The medication will be amended to include outdated medication procedures.
Standard #: 22VAC40-185-540-C Description: Based on observation and inspection of the facility, the licensee did not ensure that all required items were in the first aid kit. Evidence: During the inspection conducted on January 19, 2018, the first aid kit did not have a first aid instructional manual. Plan of Correction: The manual has been ordered and will be delivered soon.
