An unannounced monitoring inspection was conducted on January 24, 2018 from approximately 12:00 pm to 2:40 pm. Five staff were present with nineteen children. Additional staff were on site for administrative duties. Children were observed eating lunch and napping. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, special feedings, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage documentation, children & staff records, emergency supplies and required postings. Violations were reviewed with and verified by the program director during the exit interview conducted on this date.
Standard #: 22VAC40-191-60-B Description: Based on review, facility did not ensure 3 of 5 staff complete the current Sworn Statement or Affirmation form prior to beginning employment. Evidence: On January 24, 2018, the records for staff #2 (start date August 9, 2017), staff #4 (start date December 18, 2017) and staff #5 (start date December 19, 2017) do not have the current required Sworn Statement or Affirmation that went into effect July 1, 2017. Plan of Correction: Program Director will print off the new forms and have staff complete them.
