Comments:
A Subsidy Health and Safety inspection was conducted on 1/22/2018 to review supplemental health and safety requirements. There were 38 children present. The inspector arrived at 6:30 AM and departed at 8 AM. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Description: Based on review of children with diagnosed food allergy, the center failed to ensure that each child had a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. Children # 4 and 5 have a diagnosed food allergy and did not have a written allergy plan from the physician.

Plan of Correction: The director will have the parent to obtain a written plan.
