An unannounced renewal inspection was conducted today January 25, 2018 to determine the facility's compliance with licensing standards. The inspector initiated the inspection at 10:30 am and concluded the inspection at 1:30 pm. The owner/director participated with the inspection. Upon the inspectors arrival the preschool and toddler classrooms were outside on the playground. Upon returning to their classrooms, the preschool children washed their hands and gathered as a group for a story. Children also sang songs and reviewed counting. In the toddler classroom diapering and hand washing was conducted. As a group children sorted counting bears by color. Infants were observed having lunch, playing on the floor mats, and in the swing. Staff sat with children and interacted in a positive manner with the infants on the floor while the second staff feed to of the infants. Later the preschool and toddler classroom had lunch before preparing for nap time. Today's lunch consisted of macaroni and meat sauce, green beans, applesauce and milk. All areas of the facility were inspected. The playground equipment was not in use today as additional resilient materials was being installed while the inspector was on site. The facility has age and stage appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected and found complete. All required information was posted to include the license, last inspection notice, emergency numbers, menu, evacuation maps/procedures and daily schedules. The notice of civil penalty was posted at the entrance of the facility. The inspector interacted with staff and children in care. Five staff records and five children's records were reviewed. Currently medication is not being administered. One staff has current MAT training. Staff have current CPR, First aid and daily health observation training. The most recent evacuation drill was conducted on 12/18/2017. The two required shelter-in-place drill was completed on 04/21/2017 and 12/18/2017. Today's Ratios Observed: Cat 1: (22 months through 3 years) 8 children with 2 staff. Butterflies: (3-5 years) 14 children with 2 staff. Cuddle Bugs: (Infants) 6 children with 2 staff. School age children were not in attendance during today's inspection. Correction were reviewed for violations cited during the previous inspection. Standard #: 22VAC40-185-160-A Description: Based on review of staff records, the facility did not obtain documentation of a negative tuberculosis screening for staff within 21 days of employment. Evidence: Staff #4, hired 09/11/2017, had documentation of a tuberculosis screening dated 11/08/2017, exceeding the 21 day allowance to obtain. Plan of Correction: Per the owner: Staff will be required to have tuberculosis screening completed prior to state date.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of staff records, the facility did not obtain an original criminal history record report within 30 days of employment. Evidence: 1. Staff #2, hired 12/04/2017, did not have a completed original criminal history record report on file exceeding the 30 day requirement. 2. Staff #4, hired 09/11/2017, had an original criminal history record report dated 10/19/2017, exceeding the 30 day requirement. Plan of Correction: The owner called and e-mailed the agency while the inspector was on site to inquire on the status of the background checks. Per the Owner: I will be sure to send the request off at the time of hire and follow up within 30 if they are not returned.

Staff #2, hired 12/04/2017, did not have a central registry finding report on file exceeding the 30 day requirement. 2. Staff #4, hired 09/11/2017, had a central registry finding report dated 11/01/2017, exceeding the 30 day requirement. Plan of Correction: The owner called and e-mailed the agency while the inspector was on site to inquire on the status of the background checks. Per the Owner: I will be sure to send the request off at the time of hire and follow up within 30 if they are not returned.
