Revised USDA guidelines reviewed. Injury notification of injury requiring outside medication attention discussed.

An unannounced monitoring inspection was conducted on 01/26/2018 from 7:30am - 8:50am. There were 18 school age children in care with two staff. Morning program activities observed. Records reviewed for six children present. The inspector additionally reviewed medication, documentation of emergency practice drills and children's minor injuries. The violations are listed on the violation notice and were reviewed with the director at the conclusion of the inspection.

Description: Based on interview and record review, the center failed to ensure that staff emergency contact information was maintained on site. Evidence: 1. The emergency contact information for the two staff working in the program during the inspection was not on site. 2. Administrative staff verified that written emergency sheets were not on file.

Plan of Correction: Emergency contact sheets were completed during the inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method. Evidence: 1. A bottle of hand sanitizer was observed stored on the cart next to the microwave oven. a. This cart is open and accessible to children. Two children were observed playing in the area near the cart during the inspection. 2. Hand sanitizer is considered a hazardous substance as it contains 62 % ethyl alcohol.

Plan of Correction: This product will be locked up.

Standard #: 22VAC40-185-510-N
Description: Based on medication review, the center failed to return medication to the parent within 14 days. Evidence: 1. A box of children's benadryl was observed in the locked medication box. Administrative staff stated that this medication belonged to a child who does not attend this program. 2. The medication has not been returned to the parent of this child.

Plan of Correction: The medication will be returned to the parent.

Description: Based on observation, the center failed to post shelter-in-place procedures/maps in the child care program. Evidence: Shelter-in-place procedures/maps was not posted in the cafeteria area utilized by the child care program.

Plan of Correction: Written shelter-in-place procedures will be posted today.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center failed to implement a monthly practice evacuation drill. Evidence: 1. A fire drill was not conducted in December 2017. a. A lock down drill and a shelter-in-place drill was conducted in December however, these drills do not take the place of the required monthly fire drill. 2. Administrative staff confirmed that a fire drill had not been conducted in December 2017.

Plan of Correction: Going forward, fire drills will be conducted monthly.
