Comments:
An unannounced monitoring inspection was conducted today from 11:45am-2:30pm. There were 22 children directly supervised by 4 staff. The physical plant, 5 staff records, 5 children?s records, emergency supplies, and policies were inspected. No medications were present during today's inspection. Children were observed napping and eating a lunch consisting of macaroni and cheese, steamed vegetables, fruit and juice. The center's exterior has been remodeled and the playground fence has been replaced. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, not all children's records are complete. Evidence: Child 1 and Child 2's records do not contain addresses for two emergency contacts.

Plan of Correction: We will ask the parents to update this information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in a clean and safe condition. Evidence: 1. The sinks used by children throughout the center were observed with rust stains. 2. The toilet paper holders in the preschool bathroom are covered in rust.

Plan of Correction: We will put in a request to maintenance.

Standard #: 22VAC40-185-560-M
Description: Based on observation, staff were not observed to be sitting with the children at mealtime. Evidence: Staff in the preschool classroom were observed placing cots down during meal time.

Plan of Correction: I will ask the teacher to sit with the children.

Standard #: 22VAC40-191-60-C-1
Description: Based on record review, not all staff had an original criminal history record report within 30 days of employment. Evidence: Staff 1 (D.O.H. 12/19/2017) did not have an original criminal history record report on file.

Plan of Correction: A record was requested on 12/26/2017 and has not yet been returned.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, not all staff had a central registry finding within 30 days of employment. Evidence: Staff1 (D.O.H. 12/19/2017) does not have a central registry finding on file.

Plan of Correction: The request was mailed out on 12/26/2017 and we are awaiting the return.

Standard #: 63.2-1720.1-B-3
Description: Based on record review, not all staff had child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: Staff 3, did not have a central registry check on file for a state she has resided in the preceding five years.

Plan of Correction: The request will be made to the state for the central registry check will be conducted
