Comments:
A Subsidy Health and Safety inspection was conducted on 1/25/18 to review supplemental health and safety requirements. Today's visit was a routine monitoring inspection. The inspection began at 9:15am and concluded at 4:20pm. There were eighty-two children present with seventeen staff members. During the inspection classroom observations, lunch time was observed, infant care was observed, diapering was observed, ten staff records were reviewed, ten out of one hundred forty-three children's records were reviewed, emergency supplies were viewed, nap time was viewed, the emergency plan was reviewed, four of eight medications were viewed, and some additional policies and procedures were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on the inspector's review of eight out of one hundred forty-three children's records, the center failed to ensure a child with a diagnosed food allergy has a written allergy action plan from the physician indicating food allergen and what to do in the event of a suspected or confirmed allergic reaction. Evidence: Child #3 has a documented food allergy according to the child's physical exam. The is no documented allergy care plan with instructions from the child's physician.

Plan of Correction: The parents have been notified by the center with a request for an allergy action plan.
