Comments:
The licensing inspector conducted an unannounced renewal inspection from 9:30am - 1:00pm. There were twenty six preschool children in care: thirteen children in classroom one with two staff and thirteen children in classroom two with two staff and one volunteer. The children were observed at circle time, playing on the playground, playing in the centers and preparing for naps. Four staff's records and five children's records were reviewed. Standard #: 22VAC40-185-140-A
Description: Based on record reviews, the center failed to obtain documentation of a physical examination. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #2) did not contain documentation of a physical examination. Child's date of enrollment was 08/21/2017.

Plan of Correction: The teacher will contact the health and nutrition coordinator to inquire about the status of the child's physical.

Standard #: 63.2-1809-A
Description: Based on record reviews, the center failed to obtain proof of identity and age for a child's record. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #5) did not contain documentation that the child's proof of identity and age had been reviewed . Child's date of enrollment was 08/29/2017.

Plan of Correction: The teacher will follow up with the central office to see if the child's proof of identity and age has been obtained for the central office record.
