22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring visit and facility tour was conducted from 10:50am-11:40am. At the time of inspectors' arrival there were 11 preschool aged children in care with 2 staff members. Children were observed participating in free play, instructional time, transitioning between activities, and hand washing. A sample of 5 children's records and 2 staff records were reviewed. Areas of noncompliance are detailed on the violation notice and were discussed with the administration staff at the conclusion of this inspection.
Standard #: 22VAC40-185-60-A Description: Based on record review it was determined the center did not ensure that each child's record contained all of the required components. Evidence: The record for child #2(enrolled 9/6/16) did not contain the complete address for one of the two required emergency contacts. Plan of Correction: The director will request the complete address from the parent at pick up.
Standard #: 22VAC40-185-270-A Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, was maintained in a safe and operable condition. Evidence: 1. There was a small red plastic basket in the the PreK room that was cracked, presenting potential pinch points. 2. There was peeling drywall and paint in the PreK classroom along the bottom of one wall. 3. There was peeling drywall on the wall in the hallway to the school aged class. 4. There was weed barrier sticking up from the mulch, presenting potential tip hazards. Plan of Correction: 1. The basket was removed during the inspection. 2-4. Will be repaired.
Standard #: 22VAC40-185-550-E Description: Based on record review and interview it was determined the center did not maintain a record of the dates of all the practice drills for one year. Evidence: There was no documentation of Shelter-in-place and lockdown drills on the log. Classroom interviews indicated the drills were completed just not documented. Plan of Correction: The Director will create a seperate form to document Shelter-in-place and lockdown drills on.
