Technical Assistance:
Discussed the recent communication regarding fingerprint based background checks for child day programs.

Comments:
The licensing inspector conducted an unannounced monitoring inspection Thursday, January 18, 2018 from 11:30am to 2:00pm. The children were observed engaged in a variety of activities ranging from playing in the learning centers to having small group. All classrooms were inspected today. Due to snow the playground was not inspected. Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/21/2017 Last shelter-in-place drill:1/11/2017 Last fire inspection: 04/17/2017 Last health inspection: 12/18/2017 Today, the following child to staff ratios were observed: Beach Babies (Infants 6weeks-12 months)-9:3 Beach Balls (Infants 8/9 months- 16 months) -combined with the Beach Babies for the day Toddling Turtles ( Pre-Toddlers)- 8:2 Tiny Bubbles (Toddlers)- 5:2 Sunny Sailboats (Twos)- 5:1 Jumping Jellyfish 8:1 Silly Sandcrabs (Threes)- 15:2 Wacky Waves (Threes)- 9:1 Super Surfers (Pre-K)- 15:2 Lighthouse Room (Pre-K)- 10:1 Surf Shack (School Age)- 15:1 The violations from the previous inspections were checked for corrections. There was one repeat found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 01/18/2017, the center did not ensure that three of nine records reviewed contained the required information. EVIDENCE: 1. The record of Child #3 did not have documentation if the child had any allergies of intolerance to food, medication, or any other substances, and the actions to take in an emergency situation. 2. The record of Child #6 did not have documentation if the child had any chronic physical problems and pertinent developmental information and any special accommodations needed. 3. The record of Child #6 did not have documentation of the phone number for the place of employment listed for the child's father. 4. The record of Child #8 did not have documentation of the phone number for the place of employment listed for the child's father.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records on 01/18/2018, the center did not ensure that two of nine staff records reviewed contained documentation that two or more references as to character and reputation as well as competency were checked before employment. EVIDENCE: 1. The record of Staff #6, whose hire date was 07/24/2017, contained documentation of one reference. 2. The record of Staff #9, whose hire date was 12/04/2017, contained documentation of one reference.

Standard #: 22VAC40-185-550-D
Description: Based on a review of documentation and staff interview on 01/18/2018, the center did not ensure that a minimum of two shelter-in-place practice drills per year were practiced. EVIDENCE: One shelter in place drill was conducted on 01/11/2017 for the 2017 year. Staff #10 stated the second drill was not completed.

Description: Based on a review of staff records on 1/18/2018, the center did not ensure that one of nine records reviewed contained a central registry finding within 30 days of employment. EVIDENCE: The central registry findings in the record of Staff #6, whose date of hire was 7/24/2017, are dated 10/10/2017.
