Technical Assistance:
Discussed the recent communication regarding fingerprint based background checks for child day programs. Anyone who has lived in another state in the past five years is required to request a check of the child abuse and neglect registry for each state they have lived in the past five years

Comments:
The licensing inspector conducted an unannounced monitoring inspection Thursday, January 25, 2018 from 1:30pm to 4:15pm. The children were observed laying on cots for rest time and going outside to playing on the playground. The children bring their own snack and lunch from home. The school age children went on a field trip during this time. All classrooms and playgrounds were inspected today, as well as the facility?s van. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered however none is at the center. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/05/2017 Last shelter-in-place drill: 12/05/2017 (practiced monthly) Fire Inspection: 10/18/2016 Health Inspection: 10/18/2016 Total enrollment today: 19 Today, the following child to staff ratios were observed: Twos, Threes & Fours? 14:2 Fours & Fives ? 5:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records on 01/25/2018, the center did not ensure that one of five records reviewed contained a physical examination before the child's attendance or within one month after attendance. EVIDENCE: The record of Child #4, whose first day at the center was 09/11/2017, did not contain documentation of a physical examination.

Plan of Correction: Will require parent to get physical within 10 business days.

Standard #: 22VAC40-185-260-A
Description: Based on a review of documentation on 01/25/2018, the center did not ensure that a fire inspection was completed annually. EVIDENCE: The most recent fire inspection is dated 10/18/2016.

Plan of Correction: Will create an inspection binder to track expiration dates. Will have them completed by the end of next week.

Description: Based on a review of documentation on 01/25/2018, the center did not ensure that a health inspection was completed annually. EVIDENCE: The most recent health inspection is dated 10/18/2016.
