An unannounced monitoring inspection was completed on 1/25/18. There were 16 children present with four staff supervising. Five children's records were reviewed. Four staff records had been previously reviewed and were up-to-date. The inspector arrived at the school at 2:45 pm and departed at 4:30 pm. The children were observed during snack and during homework time. There was discussion with the site director and staff about the following: documentation of emergency drills.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that all required information was completed for each child's record. Evidence: 1. The record of child #1 did not have the name, address, and phone number of a second emergency contact. The emergency contact listed was the parent. The emergency contact is required to be someone other than the parent. 2. The record of child #3 was missing the work phone number for the father. Plan of Correction: The staff will get the information completed.
Standard #: 22VAC40-185-340-D Description: Based on observation and interview, the center failed to ensure that in each grouping of children at least one staff member who meets the qualifications of program leader or program director regularly present. Evidence: 1. At arrival of the inspector at 2:45 pm, there were 16 children observed in the cafeteria with one staff person. The children arrive in the cafeteria at approximately 2:35 pm according to the staff. This staff person was a program aide and not qualified as a program leader or program director. Plan of Correction: This was corrected during the inspection as a program leader arrived at 3:00 pm. The program director arrived at approximately 3:15 pm. The normal schedule is for the program director to be present when the children arrive daily.
