An unannounced renewal inspection was conducted with the center director and assistant director from 1:30 - 4:15 pm. Five children, ranging in age from three to five years old, were present with one staff member supervising. The children were observed napping during this inspection. Lunch today was cheeseburger, salad, cantaloupe and milk. Five children?s records and five staff records were reviewed. The last emergency escape drill was 1/29/18. No medications were on site. Standard #: 22VAC40-185-140-A Description: Based on a review of five children?s records, the center did not obtain documentation that each child has received a physical examination before the child?s attendance or within one month after attendance. Evidence: Child B?s file (date of enrollment: 8/21/17) did not have documentation of a physical examination. Plan of Correction: Parents have been notified that information must be submitted to continue enrollment.
Standard #: 22VAC40-185-60-A Description: Based on a review of five children?s records, it was determined that not all records were complete. Evidence: Child A?s file (date of enrollment: 8/21/17) did not have documentation of proof of age and Child B?s file (date of enrollment: 8/21/17) did not have the address for one of the two emergency contacts. Plan of Correction: Parents have been notified that information must be submitted to continue enrollment.

Evidence: Staff 1 (date of hire: 9/11/17), Staff 2 (date of hire: 10/30/17) and Staff 3?s (date of hire 8/21/17) files did not contain documentation of two references as to character and reputation as well as competency before employment.
Plan of Correction: Already corrected.
