Technical Assistance:
New background check guidelines and procedures, which can be found on the state website, were discussed with the center director during the inspection.

Comments:
An unannounced monitoring inspection was conducted from 11:15am until 2:20pm. At the time of the tour, there were 50 children in care with 8 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed participating in various class activities and eating lunch. Infant diapering, hand washing, medication, transportation, first aid and emergency supplies, the emergency preparedness plan and required center postings were reviewed. Violations in the areas of administration and special care provisions and emergencies are identified on the violation notice and were discussed with the center director during the exit interview.

Standard #: 22VAC40-185-130-A
Description: Based on record review and interview, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record for child #1 does not contain documentation of the required immunizations. a. Child #1 has a date of enrollment of 11/20/17 and was in care on the day of the inspection. 2. The center director confirmed that the record for child #1 does not contain documentation of immunizations.

Plan of Correction: The center responded with the following: Child was unable to return without immunizations. Has been received.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's first day of attendance or within one month after attendance. Evidence: 1. The record for child #1 does not contain documentation of a physical examination. a. Child #1 has a date of enrollment of 11/20/17 and was in care on the day of the inspection. 2. The center director confirmed that the record for child #1 does not contain documentation of a physical examination.

Plan of Correction: The center responded with the following: Child was unable to return without physical. Has been received.

Standard #: 22VAC40-185-160-C
Description: Based on record review and interview, the center did not ensure that at least every two years from the date of the first initial screening or testing staff members shall obtain and submit the results of a follow-up tuberculosis screening. Evidence: 1. The most recent tuberculosis screen for staff #1 is dated 10/25/13. Staff #1 was caring for children on the day of the inspection. 2. The center director confirmed that the last documented tuberculosis screening for staff #1 is dated 10/25/13.

Plan of Correction: The center responded with the following: Staff has gone for TB test. Results will be read on 1/31/18.

Standard #: 22VAC40-185-510-C
Description: Based on medication review and interview, the center did not ensure that procedures for administering medication shall include methods to prevent use of outdated medication. Evidence: 1. The EpiPen for child #2 has an expiration date of 11/17. 2. The albuterol inhaler for child #3 has an expiration date of 6/17. 3. The center director confirmed that the medications for child #2 and child #3 were expired.

Plan of Correction: The center responded with the following: Epipen was sent home on 1/26/18. Inhaler sent home on 1/26/18. New current inhaler was received. Epipen will be sent in.

Standard #: 22VAC40-185-520-C
Description: Based on observation, the center did not ensure that if diaper ointment or cream is used the product shall be labeled with the child's name. Evidence: A tube of Neosporin Eczema Cream for child #1 was not labeled with the child's name.

Plan of Correction: The center responded with the following: Cream was labeled on the same day.
