Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Disclosure Statement for new staff hired after 7/1/2017 and requesting out of state Central Registry checks for staff who have lived outside the State of VA within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out of state central registry forms. VDSS will be paying for initial child care fingerprint checks until 9/20/18. Each facility must establish a point of contact with DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligible letters will be sent out by emails and staff may not work with children (ratios or supervision) until a completed letter is received. Please read, review and respond to all correspondences regarding the fingerprint roll out.

Comments:
An unannounced renewal study was conducted today. Forty seven children were in care with direct care staff in ratios for mixed age groups in two areas and a younger classroom. Space is equipped with Montessori equipment and tools for children to use. Children were observed transitioning to outside play time, working in centers, preparing for lunch and circle time. Lunch served today was Shepherd's Pie, slaw and fruit. Several emergency medications are on-site and authorizations are current for long term use. Three staff have MAT. Staff records are current as well as emergency preparation. Areas of non-compliance are identified in the violation notice. Description: Based on record review, 1 of 5 children's immunization record is not in the record to verify that the child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: child #2 did not have a completed immunization record in the file to verify the required immunizations are up to date.

Plan of Correction: The immunization record will be requested and placed in the child's record.

Description: Based on record review, 1 of 5 children's records was incomplete. Evidence: Child #1 was missing complete addresses for 2 emergency contacts.
