Comments:
An unannounced monitoring inspection was completed on 1/31/18. There were 27 children present with three staff supervising. Five children's records were reviewed. Four staff records were previously reviewed and were up-to-date. The inspector arrived at the school at 3:05 pm and departed at 4:45 pm. The children were observed during homework time, during snack, during gym time, and during departures.

Violations:
Standard #: 22VAC40-185-60-A
Description: REPEAT VIOLATION Based on record review, the center failed to ensure that the children's records contained complete information as required by the Standards. Evidence: 1. The father listed on the birth certificate of child #2 was listed as not authorized to pick up the child. There was no legal paperwork to support this. 2. The records of children #6 and #7 (siblings) were missing a complete address for one of two emergency contacts.

Plan of Correction: The site director will have the parents complete the records.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the center failed to ensure that children under 10 years of age were within actual sight and sound supervision of staff. Evidence: 1. There was a five year old child who was observed leaving the cafeteria, going through the doors into the hallway to the water fountain. The water fountain was not within sight of any staff and the child was not able to be heard by staff. This was during homework time; the noise level in the cafeteria would not have allowed the child to be heard. Three staff were observed to have their backs to the door and one staff person was talking with a parent. No staff were aware of the child leaving the cafeteria. The child was out of sight and sound supervision for approximately two minutes.

Plan of Correction: The site director will talk with staff about repositioning themselves to be able to supervise the door to exit the cafeteria.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to ensure that the children washed their hands as required. Evidence: 1. The children were observed during snack. The children did not wash their hands or use disposable wipes after eating.

Plan of Correction: The site director will remind staff to use disposable wipes after eating each day.
