VENDSUB-000 STAFF QUALIFICATIONS AND TRAINING
VENDSUB-000 SPECIAL SERVICES
A Subsidy Health and Safety inspection was conducted on September 22, 2017 to review supplemental health and safety requirements. Licensing Inspector arrived at 3:03pm and departed at 4:35pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 25 school age children present with two staff members. Children were observed arriving to the program, having snack which consisted of applesauce and pretzels, doing homework and playing with various tabletop activities to include journaling, arts and crafts and Legos and blocks. The sample size consisted of five children's records. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-004-F Description: Based on observation and inspection of the facility, the vendor did not ensure that the allergy list was current with all children's allergies, sensitivities, and dietary restrictions documented. Evidence: A review of children's record (Child #1) indicated that the posted allergy list did not include all children's allergies and sensitivities. Plan of Correction: The list was updated while the inspector was present.
Standard #: VENDSUB-000-006-B-11 Description: Based on review and inspection of the facility, the vendor did not ensure that a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction was obtained. Evidence: There was not an allergy care plan for Child #2 who had a diagnosed food allergy indicated in the record by a physician. Plan of Correction: Site Director will contact the Area Manager regarding the child's enrollment status. If applicable, an allergy plan will be obtained.
