Technical Assistance:
Background check regulations were discussed with the program director. Documentation of program leader qualifications was discussed with the program director. Potential hazard of using stick pins in an infant classroom was discussed with the program director

An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 12:30 PM and departed at 3:10 PM. Upon arrival of the inspector, a total of nineteen children were in care with three staff. Nap time, afternoon snack service and afternoon activities were observed. A sample size of five staff records and five children's records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Standard #: 22VAC40-185-130-B
Description: Based upon review of five children's records and staff interview, the facility has not ensured that documentation of additional immunizations is obtained once every six months for children under the age of two years. Evidence: 1. The record provided for child 5, born 2/4/2017, does not have documentation of updated immunizations since 2/7/2017. 2. Staff 1 was unable to provide more recent documentation of immunizations for child 5.

Plan of Correction: The facility responded with the following: We will create a method to track immunization updates for children under age 2 years.

Standard #: 22VAC40-185-270-A
Description: Based upon observation, the facility has not ensured that equipment inside and outside is maintained to be in a safe condition. Evidence: There is a green and yellow plastic wagon on the playground. One side railing of the wagon is cracked, creating a potential pinching hazard. The second side railing is broken, revealing sharp edges that could cut a child.

Plan of Correction: The facility responded with the following: The wagon will be disposed. It will be discussed with staff that broken items must be noted when conducting the playground inspection and brought to the attention of the director for repair or disposal.

Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by the children. Evidence: 1. There is a container of Klar Danvers pump hand soap on the sink in the girls' bathroom. The soap is labeled "warning" and "keep out of reach of children". 2. Staff 1 verified that the container is Klar Danvers hand soap with a warning label.

Plan of Correction: The facility responded with the following: The wall dispenser will be replaced. In the interim soap without warning labels will be purchased and used for handwashing.

Standard #: 22VAC40-185-350-C
Description: Based upon observation and staff interview, the facility has not ensured that when children are in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group applies. Evidence: 1. At 12:30 PM, there was a grouping of eleven children ages 3 and 4 years sleeping on cots. In the same grouping with one staff person (staff 7) there were an additional four school aged children participating in activities at the table. Thus there were a totla of 15 children in care with one staff person. A second staff person did not arrive until 12:55 PM. 2. Staff 7 stated that she thought the second staff person required was in the foyer area. No additional staff person was in the building until 12:55 PM.

Plan of Correction: The facility responded with the following: Nap time ratio was not clearly understood. If children are combined and not all children are sleeping, the ratio required for the youngest child in the grouping will be followed.
