Technical Assistance:
The following was discussed with the owner: When an employee indicates on the Sworn Statement of Affirmation, living in another state within the past 5 years A Central Registry check in the Virginia and the other state lived in is required.

Comments:
An unannounced monitoring inspection was conducted from 1:00pm-3:00pm. Upon arrival there were 53 children in care with 11 staff present. A sample of 2 staff and 4 children's records were reviewed in addition to 3 medications. Children were observed during the designated rest period, watching television and getting up from nap. Infants were observed playing on the floor with staff.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: The wooden door between the three year-old room and pre-k room was broken at the bottom of the door. The broken area of the wood was splintered.

Plan of Correction: The provider gave the following "Plan of Correction:" The owner will have the door assessed and either repaired or replaced. In the meantime the owner will put duct tape over the area until it is resolved.

Standard #: 22VAC40-185-330-B
Description: Based on observation and interview the center failed to ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: On the playground there were 2 uneven bars that did not have any resilient surfacing. The owner stated the children use the bars for climbing and flipping.

Plan of Correction: The provider gave the following "Plan of Correction:" The owner will put caution tape on the 2 bars and will not be used by children in care until a decision is made about whether the bars will be removed or resilient surfacing will be added to the area.
