Discussed with the Director: Using a Junior staff person at the door to the gym to assure that children do not go in and out while the other staff person is participating in physical activities with the children.

An unannounced monitoring inspection was conducted beginning at 3:40 and ending at 6:15. There were 30 children present, all school age. There were 6 staff present. Children were observed upon arrival, having after school snack of turkey, cheese, pita and pears and milk.; having physical activity in the gym and interacting with two local police officers who came to visit with the children. Emergency escape drills and shelter in place drills are up to date as are health and fire inspections. Reviewed staff and children's files - see Violation notice. Description: Based on interview with staff and a review of a staff file, it was determined that there was no documentation of staff agreement to disclose moving violations during the time they are driving for the facility. Evidence: Staff A who drives for the facility and takes children to and from school in the facility vehicle does not have a signed agreement that he will notify the facility of any moving violation that he receives.

Plan of Correction: All staff who drive for the agency will sign a statement that the Branch Director will draft regarding staff responsibility.

Description: Based on interview with the Director, it was determined that not all staff who transport children have current First Aid and CPR. Evidence: Staff A, who drives children to and from school does not have First Aid and CPR training.

Plan of Correction: The staff person will not be allowed to drive until they have current First Aid and CPR.
