Technical Assistance:
We discussed the requirements for children's records. The risk assessment for a violation was provided. Also, we reviewed diapering procedures and staff positioning disposal containers so they can easily access the foot pedals.

Comments:
Thank you for your assistance during today's unannounced renewal inspection conducted from 10:15 AM to 4:00 PM. Today, there were eighty-three children, ages infancy through five years, with fourteen staff. I viewed program activities, daily routines, staffing, supervision, interactions with children, equipment, posted information, insurance, agency inspections reports, infant care and records, diaper ointment, nutrition/menu, emergency supplies and drill records, injury records, eight records for children, eight staff records, staff qualifications, staff training, indoor and outdoor areas. The children have enjoyed seasonal activities such as recent Valentine art. Preschool age children received opportunities for group discussions and individual choices for activities and use of classroom toys, books and equipment. Infant staff were provided individual attention to meet their needs for comfort and feeding. The older infants enjoyed tunnel and tent activities in the room. The playgrounds were not in use this date due to wet and icy conditions. Resilient surfacing depths at fall zones of outdoor equipment complied with licensing requirements. Standard #: 22VAC40-185-140-A
Description: Based on a review of eight records for children, and an interview with administrative staff, the physical examination report for child 1 was obtained late and not within the first month of attendance. Evidence: The start date in the file was October 16, 2017 and the physical examination information was received by FAX on December 1, 2017.

Plan of Correction: Administrative staff will require the physical examination records for new children no later than the first month of attendance if the reports are not provided at the start of attendance. The director will use a reminder system for determining the need for information.
