Comments:
A licensing inspector and the licensing administrator conducted an unannounced monitoring inspection Tuesday, January 23, 2018 from approximately 9:30AM to 12:00PM to monitor the provider's provisional license status. The following ratios were observed with a total of 21 children present during the inspection:
Early Head Start (1 to 2 years of age) - 7 children with 2 staff
Head Start (Three & Four years of age) - 13 children with 1 staff - see violation notice
Preschool I (Infant ? One year of age) - 7 children with 2 staff
Preschool II (Two ? Four years of age) - 15 children with 2 staff
School Age children were not present during the inspection.

During the inspection, children and staff were observed playing with toys, looking at books, working puzzles, spelling their names, singing songs and building with blocks. During the inspection, five children's and two staff records were reviewed.

Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: 22VAC40-185-160-A
Description: Based on the review of two staff records, the facility did not obtain a negative tuberculosis screening for one staff member within the required time frame. Evidence: During the inspection, the licensing inspector and licensing administrator observed the record for Staff #2 contained a documented hire date of 12/18/17. Based on the 12/18/17 hire date, a negative tuberculosis screening was required to be obtained by 01/08/18. A member of management confirmed Staff #2 completed orientation on 12/18/17.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed an aerosol can of disinfectant sitting on top of a paper towel dispenser located in the Head Start Classroom. The paper towel dispenser was attached approximately five feet up the wall by the classroom bathroom.

Description: Based on observation, the facility did not ensure all required electrical outlets contained protective covers. Evidence: During the inspection, the licensing inspector and licensing administrator observed an electrical outlet without protective covers located in the hallway of the facility.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation and interview, the facility did not ensure the required staff-to-child ratio for children three and four years of age. Evidence: The licensing inspector and licensing administrator arrived at the facility at 9:31AM. Staff #3 assisted with the inspection and walked with the licensing inspector and licensing administrator while observations of classrooms were conducted. At 9:51AM, Staff #1, the licensing inspector and licensing administrator entered the Head Start Classroom. Staff #4 was alone, supervising 13 children (3 to 4 years of age). Staff #3 was asked what staff was assigned to the Head Start Classroom. Staff #3 then stated due to a staff absence, Staff #3 was assigned to be in the Head Start classroom for the day. The required staff-to-child ratio for children three and four years of age is one staff member for every 10 children.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of two staff records, the facility did not obtain a central registry finding within 30 days of employment for one staff member, but the staff is still working.

Evidence:
During the inspection, the licensing inspector observed the record for Staff #2 did not contain a central registry finding. The documented hire date for Staff #2 was listed as 12/18/17. A copy of the submitted central registry form was documented as being mailed 01/12/18. Based on the 12/18/17 hire date, a central registry finding was required to be obtained by 01/17/18. A member of management confirmed Staff #2 completed orientation on 12/18/17. Central registry findings are required to be requested within seven days of being hired and must be obtained within 30 days of hire. If the facility does not have a central registry finding for a staff within 30 days of employment, the center must deny continued employment until the finding is received.

Description: Based on observation, the facility did not have the most recent inspection summary / violation notice posted on the premises. Evidence: During the inspection, the licensing inspector and licensing administrator observed the inspection summary/violation notice posted in an office at the front of the facility. The posted inspection was dated 05/31/17. The most recent inspection was conducted on 11/17/17.
