- Technical Assistance:
A Subsidy Health and Safety inspection was conducted on Wednesday, February 7, 2018 from approximately 10:00AM to 2:00PM to review supplemental health and safety requirements. The following ratios were observed with a total of 33 children present during the inspection:
Infants - 4 children with 1 staff
Toddlers - 4 children with 1 staff
Twos - 8 children with 1 staff
Threes - 7 children with 1 staff
Fours & Fives - 10 children with 1 staff
School Age children were not present during the inspection.
During the inspection, children were observed sleeping, playing with toys, coloring and having books read to them. All areas of the facility including classrooms, hallways, bathrooms and kitchen were inspected. The facility was equipped with toys and supplies and items were available to the children. During the inspection, five children's records and five employees? records were reviewed.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11 Description: Based on review of five children's records, the facility did not obtain a written allergy care plan for a child with a diagnosed food allergy. Evidence: During the inspection, the licensing inspector observed the record for Child #1 indicated the child has a food allergy. The record did not contain a written allergy care plan that also included instructions from a physician. Plan of Correction: We will request this information from the parent.
Standard #: VENDSUB-000-012-C Description: Based on the review of five staff records, one staff has not completed the Virginia Preservice Training within the required time frame.
During the inspection, the licensing inspector observed the record for Staff #2 (hire date 09/05/17) did not contain documentation of completing the required Virginia Preservice training. A member of management stated the staff had begun the training but has not completed it.
Plan of Correction: Staff will complete the training by Monday.
Standard #: VENDSUB-000-012-E Description: Based on review of five staff records, two staff members did not obtain first aid and CPR certfication within 30 days of employment. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 11/13/17) and Staff #2 (hire date 09/05/17) did not contain documentation of first aid and CPR certification. A member of management confirmed during the inspection the staff had not completed the training and stated the training is scheduled for 02/17/18. Plan of Correction: Training scheduled for 02/17/18
