A Subsidy Health and Safety inspection was conducted on 02/07/18 to review supplemental health and safety requirements. Licensing Inspector arrived at 10:00am and departed at 12:30pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 53 children in care with 16 staff present. Children were observed on the playground, participating in dramatic play and during lunch. A sample of 3 staff and 6 children's records were reviewed. The center had no medications at the time of inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-006-B-11
Description: Based on a record review and interview with the director, the vendor failed to ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child #6 had a food allergy listed on the "Allergy and Dietary Preference" sheet provided by the director. The vendor did not have an allergy action plan for the child.

Plan of Correction: The vendor provided the following plan of correction: The director will call the child's parent today and request a written allergy action plan from the child's doctor.

Standard #: VENDSUB-000-012-E
Description: Based on a record review the vendor failed to ensure all staff had a current certification is CPR/First Aid. Evidence: Staff # 1 (doh 10/30/17) did not have documentation of having a CPR/First aid certification.

Plan of Correction: The vendor provided the following plan of correction: Staff #1 is registered to take a CPR/First Aid class at the end of February. All staff will be renewed at this time as well.
