VENDSUB-000 STAFF QUALIFICATIONS AND TRAINING
A Subsidy Health and Safety Inspection was conducted on February 6, 2018 to review supplemental health and safety requirements. The Licensing Inspector arrived at 3:40am and departed at 5:00pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour, there were 13 school age children present with two staff members. The licensing inspector observed the children being escorted to the restroom, having snack which consisted of cereal and milk, doing homework, playing with blocks and talking freely amongst themselves. The sample size was five children's records. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. The exit interview was conducted with the Site Director.
Standard #: VENDSUB-000-006-B-3 Description: Based on record review, in five of five children's records reviewed, the vendor did not ensure that all required parent contact information was in each child's record. Evidence: There was no documentation of the work address for one or both parents. Plan of Correction: The Site Director will have the parents to update the enrollment forms to include the work addresses.
