There should be documentation of the names of any additional programs or schools that the child is concurrently attending and the grade or class level.
An unannounced renewal inspection was conducted on February 7, 2018. At the time of entrance there were 2 staff present with 11 preschool age children; an additional staff person arrived during the course of the inspection. The sample size consisted of five children's records and five staff record. Children were observed participating in circle time, singing and during outdoor play. Licensing inspector reviewed supervision, administration, nutrition, indoor and outdoor areas, and health and safety. No medications are being administered or kept at the center as verified by the Program Director.
Standard #: 22VAC40-185-130-A Description: Based on record review, in one of five children's records reviewed, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: During the inspection conducted on February 7, 2018, there was no documentation of immunizations in the record for Child #2. Plan of Correction: The parent will be contacted to obtain a copy of the immunizations.
Standard #: 22VAC40-185-60-A Description: Based on record review, in five of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on February 7, 2018, the following information was not documented in each child's record: 1. The records for Child #2, Child #3, Child #4 and Child #5 did not have start dates. 2. The record for Child #1 did not have an address for one of two required persons to contact in the event of an emergency where the parent cannot be reached. 3. The record for Child #2 did not have complete addresses for two persons to contact in the event of an emergency where the parent cannot be reached. Plan of Correction: The start dates will be added and the addresses will be obtained.
Standard #: 22VAC40-185-70-A Description: Based on record review, in four of five staff records reviewed, the licensee did not ensure that each staff record was documented with all required information. Evidence: During the inspection conducted on February 7, 2018, the records for Staff #1, Staff #2, Staff #3 and Staff #4 did not have documentation of the address of one person to contact in the event of an emergency. Plan of Correction: The emergency contact addresses will be added.
Standard #: 22VAC40-185-550-D Description: Based on observation and inspection of the facility, the licensee did not ensure that the center shall implement a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection conducted on February 7, 2018, there was documentation of one of two required shelter-in-place drills practiced in 2017. Plan of Correction: Two drills will be conducted per calendar year.
Standard #: 22VAC40-191-60-B Description: Based on record review, in two of five staff records reviewed, the licensee did not ensure that the current sworn statement or affirmations were completed for staff hired after July 1, 2017. Evidence: During the inspection conducted on February 7, 2018, the outdated version of the sworn statements or affirmations were completed for Staff #2 and Staff #4. Plan of Correction: The sworn statements will be completed today.
