An unannounced monitoring inspection was conducted from 2:15 p.m.-4:30 p.m. with the center director. 59 children, ages 10 months through 5 years old, were under the care and supervision of eight staff. Children were observed eating afternoon snack and listening to a readaloud. Afternoon snack served today was graham crackers and apple slices. Required information was posted. Both the fire and health inspections are current. The last emergency escape drill was 1/17/18; the last lockdown drill was 10/11/17; and the last shelter-in-place drill was 12/8/17. One medication, five children's records, and five staff records were reviewed. A subsidy health and safety inspection (SHSI) was also conducted (see separate report). For any questions about this inspection, please contact licensing inspector Marty Quade at (540) 272-5065 and Marty.Quade@dss.virginia.gov.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of five staff records, the center did not have an original central registry report within 30 days of employment of a staff member. Evidence: Staff A's (hired 11/9/17) central registry report was dated 12/19/17. Staff B's (hired 5/1/17) central registry report was dated 6/6/17. Staff C's (hired 10/31/16) central registry report was dated 12/13/16. Plan of Correction: The central registry timeline will be followed more closely, particularly the documentation of the delays with VDSS in processing this background check.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
