A Subsidy Health and Safety Inspection was conducted on February 8, 2018 from approximately 10:30am to 2:30pm to review supplemental health and safety requirements. Upon arrival to the facility, there were 25 children present with five staff members. Additional staff were on site for administrative and support purposes. The following items were reviewed as a part of the inspection: indoor areas, supervision, administration, health and safety, physical plant, required documentation, and emergency supplies, procedures and required postings. Four medications were reviewed. The center does offer transportation services. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-005-B Description: Based on observation and inspection of the facility, for each group of children, the vendor did not maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: For each group of children present (four groups), there was not a written record of daily attendance that documented the arrival and departure of each child in care as it occurred. Plan of Correction: There will be a roster in each room for each class.
Standard #: VENDSUB-000-006-B-3 Description: Based on record review, in four of five children's records reviewed, the vendor did not have all required parent contact information documented. Evidence: The records for Child #1, Child #2, Child #4 and Child #5 did not have complete work addresses for one or both parents. Plan of Correction: The complete work addresses will be added.
