Comments:
An unannounced monitoring inspection was conducted from 2:00pm-3:20pm. At the time of entrance, 34 children were in care with 2 staff members present. Children were observed during check in procedures, snack time, free play and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, one of five children's records did not have complete information. Evidence: 1. Child E's record was missing one parent's work phone number.

Plan of Correction: I will ask the parent for the information and update the record.

Standard #: 22VAC40-185-510-G
Description: Based on review of one medication, the center did not have written authorization to administer medication to a child in care. Evidence: 1. The center had 1 medications for Child F that did not have parent authorization. The form was signed by the doctor, but was not signed by the parent to give authorization.

Plan of Correction: I will ask the parent to sign the medication authorization form.
