Comments:
A Subsidy Health and Safety monitoring inspection was conducted on 2/9/2018 to review supplemental health and safety requirements. This inspection began at 9:50 am and ended at 1:00 pm. There were 7 staff with 31 children in 5 classrooms at the time of the visit. At the start of the inspection, children were observed painting, working on math activities and playing on the playground. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were no medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Standard #: VENDSUB-000-005-B
Description: Based on record review and interview, the vendor does not record the arrival and departure time for each child. Evidence: The attendance sheet that the vendor uses does not record arrival and departure times for each child. The center's assistant manager confirmed that they do not record the times of the children's arrival and departure.

Plan of Correction: I will change the attendance list.

Standard #: VENDSUB-000-006-B-11
Description: Based on record review, a written allergy care plan including instructions from a physician, was not provided for each child with a diagnosed food allergy. Evidence: Child #7 and Child #8 are each diagnosed with food allergies and have emergency medication kept on site. None of these children have documentation of an allergy care plan from their physicians.

Plan of Correction: I will talk to the parents.

Standard #: VENDSUB-000-012-B
Description: Based on record review and interview, one employee has not completed the Virginia Preservice Training within the first 90 days of employment. Evidence: Staff #2 (date of hire: 5/13/2017) does not have documentation of having completed the Preservice training. The center director confirmed that this employee has not completed the training.

Plan of Correction: I will have her get it done.

Standard #: VENDSUB-000-012-F
Description: Based on record review and interview, one staff member has not completed CPR and first aid training within the first 90 days of employment. Evidence: Staff #2 (date of hire: 5/13/2017) does not have documentation of having completed CPR and first aid training. The center director confirmed that this employee has not yet completed the training.

Plan of Correction: I will find out when there is another class and get her registered.
