Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, February 06, 2018 from 10:33am to 2:30pm. The infants were observed playing on the floor with their teachers and being fed. The toddlers were observed playing with toys, eating lunch and later resting on mats. Lunch today consisted of meatballs, green beans, pineapples and milk. The menu was posted. All classrooms and playgrounds were inspected today. Five children?s records and five employee records were reviewed during this inspection. The center administers medication and it was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 01/15/2018 Last Shelter-in-place drill: 05/25/2017 Last fire inspection: 08/28/2017 Last health inspection: 06/30/2017 Total enrollment today: 34 Today, the following child to staff ratios were observed: Infant I (6 weeks to 12 months) ?11:4 Infant II (12-14 months) ?7:2 Toddler I (14-18 months)-7:2 Toddler II (18-24 months)- 8:2 School Age ?not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-130-B
Description: Based on a review of children's records on 02/06/2018, the center did not ensure that three of five records reviewed contained documentation of additional immunizations once every six months for children under the age of two years. EVIDENCE: 1. The only documentation of immunizations in the record of Child #1 is dated 05/04/2017. 2. The only documentation of immunizations in the record of Child #2 is dated 07/26/2017. 3. The only documentation of immunizations in the record of Child #5 is dated 07/14/2017.

Plan of Correction: Will give a written notice today for parent to have immunizations by Friday, 2/9/2018. Will review all files and make a chart.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records on 02/06/2018, the center did not ensure that one of five records reviewed contained documentation of a physical examination before the child's attendance or within one month after attendance. EVIDENCE: The record of Child #3, whose first day at the center was 10/23/2017, did not contain documentation of a physical examination.

Plan of Correction: Will notify parent today and have them bring it by Friday.

Standard #: 22VAC40-185-520-C
Description: Based on observation an staff interview on 02/06/2018, the center did not ensure that if over the counter diaper product is used that the products shall be in the original container and labeled with the child's name. EVIDENCE: 1. An unlabeled container of diaper powder was observed in Bathroom 1. 2. According to Staff #6 the contents of the container was cornstarch 3. According to Staff #5 the product had been used on the child.

Plan of Correction: Will label with the child's name and what the product is today.

Standard #: 22VAC40-185-520-D
Description: Based on observation an staff interview on 02/06/2018, the center did not ensure that if insect repellent is used that the products shall be in the original container and labeled with the child's name. EVIDENCE: 1. Two unlabeled bottles of insect repellent were observed in Bathroom 3. 2. According to Staff #5 the products had been used.

Plan of Correction: Will label with the child's name today.

Standard #: 22VAC40-185-550-E
Description: Based on a review of documentation and staff interview on 02/06/2018, the center did not ensure that a record of the dates of the practice drills were maintained. EVIDENCE: 1. The only documentation of a shelter in place drill for 2017 that was noted was dated 05/25/2017. 2. Staff #6 stated the drill was done in December 2017, just not documented.

Plan of Correction: Corrected on site. Drill documented.

Standard #: 22VAC40-185-570-E
Description: Based on observation on 02/06/2018, the center did not ensure that prepared formula was dated. EVIDENCE: 1. Prepared bottles belonging to Child #6 were observed in the refrigerator in the infant room. The bottles were not dated. 2. Prepared bottles belonging to Child #7 were observed in the refrigerator in the infant room. The bottles were not dated. 3. Prepared bottles belonging to Child #8 were observed in the refrigerator in the infant room. The bottles were not dated.

Plan of Correction: Will date bottles today. Will make sure that it is done when children check in.
