An unannounced monitoring inspection was conducted on 2/9/18 from 10:15am - 12:15pm. When the Licensing Inspector arrived there were 56 children ages two years old to five years old in care with 17 staff. Children were observed participating in various activities in the classrooms, playing outside, eating snack and participating in a special event function with their grand parents. Records were reviewed for six children and six staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-130-A Description: Based on a review of six children's records, it was determined that the facility did not ensure that the center obtains documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. The record for child #6, present during the inspection, did not contain documentation of an immunization record. 2. Staff #7 (Assistant Director) reviewed the record for child #6 and confirmed there was no immunization record available for viewing during the inspection. Plan of Correction: The facility responded: The parents of child #6 will be asked to provide a copy of a current immunization record. All newly enrolled children will not start unless they have provided an immunization record.
Standard #: 22VAC40-185-140-A Description: Based on a review of six children's records, it was determined that the facility did not ensure that the center obtains documentation of a physical examination by or under the direction of a physician before the child's attendance, or within one month after attendance. Evidence: 1. The record for child #6 (date of enrollment 1/2/18), present during the inspection, did not contain documentation of a physical examination. 2. Staff #7 (Assistant Director) reviewed the record for child #6 and confirmed there was no physical examination available for viewing during the inspection. Plan of Correction: The facility responded: The parents of child #6 will be asked to provide a copy of a physical examination. All newly enrolled children will have documentation of a physical examination in their record within one month of attendance.
