Comments:
A Subsidy Health and Safety inspection was conducted on 2/12/18 to review supplemental health and safety requirements. There were 43 School-age children present with three staff. Ten children records reviewed. Five Medications checked. No staff files on site. Conversations were held with several staff and children. One classroom observation conducted. A licensing inspection was also conducted this dated; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental health and safety inspection and are identified on the violation notice.

Description: Based on review of children records the center has not received all required information for children with food allergies. Evidence: On 2/12/18, there were four children that have food allergies enrolled in the program, and there were no allergy plans available in files.

Plan of Correction: Per director, will email and speak to parents in person to request the allergy action plans.
