An unannounced inspection was conducted from 9-11 am. The Inspector observed classes, interviewed staff, reviewed records, and checked medication authorization/administration procedures. Hand washing and special care was also observed. Children were observed in circle time, writing activity, creative art projects, and play. The Inspector did not observe outside due to the rain. Standard #: 22VAC40-185-60-A Description: Based on record review of children's files it was determined the center did not have complete information in one file. Evidence: 1. 1 of 8 files reviewed did not have address for one emergency contact. Plan of Correction: We are keeping the file to have parent fill out tonight.
Standard #: 22VAC40-185-270-A Description: Based on observation of areas and equipment, the center is not maintaining items in a clean and safe condition. Evidence: 1. Inspector observed ripped window screens in the two's class and the Preschool room. 2. The Inspector observed the soft couch in the Two's class was ripped and the blue chairs in the Pre K room were ripped. 3. The Two's class had broken Lego panels glued on a table and many toys and structures had not been cleaned as evident by crayon markings and dirt around the shelves. Plan of Correction: We will contact maintenance abut repairs and make necessary corrections on other items.
