Discussed the importance of ensuring that medications accepted from a parent is not expired.
An unannounced Renewal inspection was conducted this date. A total of 23 children were present with 3 staff. At the beginning of the inspection the children were in the gym rehearsing their Olympics program scheduled later this week. A variety of performances were observed which included dancing, basketball skill and hula hooping. A nice transition was observed from the gym, to the bathroom to handwash before snack. Snack today was milks and graham crackers. Children transitioned to the cafeteria for snack and was observed completing homework, reading and talking in small groups. Positive interaction was observed between children and staff. Some violations found today, one of which is a repeated violation from the last inspection in December. A total of 6 children's records and all medication were reviewed. Note: Staff files are reviewed on an annual basis by representatives from the Commonwealth of Virginia, Department of Social Services, Division of Licensing, Fairfax Licensing Office and through the Loudoun County YMCA administrative offices located in either Reston and/or Leesburg, Virginia. Any observed standards found to be out of compliance would be cited in a violation notice issued at that time. Verification of CPR/First Aid, MAT training and daily health observation training was verified during this inspection. This inspection began at 3pm and ended at 5pm. Standard #: 22VAC40-185-60-A Description: Based on child record review, four out of five child records were incomplete. Evidence: Child #3, 4 and 5's record did not contain documentation of proof of identity. Child #1's second emergency contact did not include an address. Child #3's record did not have a second emergency contact which included a name, address and phone number. Plan of Correction: We will obtain this information by the end of this week and update the record.
Standard #: 22VAC40-185-70-A Description: Based on review of staff information and staff interviews, staff information was not maintained. Evidence: No emergency contact information for the three staff present was maintained. Plan of Correction: We will document and maintain staff information to include emergency contacts for each.
Standard #: 22VAC40-185-510-A Description: Based on review of the medication on site, authorization for one medication was not obtained. Evidence: Child #6 has an EpiPen and ProAir inhaler on site. A parent and physician authorization was not obtained. Plan of Correction: We have requested the authorization from the parent on numerous occasions. If we do not receive an authorization from the parent we will return the medication to the parent.
Standard #: 22VAC40-185-510-C Description: Based on review of the medication on site, center did not follow their medication policy to prevent outdated medication. Evidence: Child # 6's EpiPen expired in December 2017. Plan of Correction: We will return this medication today and request a new one.
Standard #: 22VAC40-185-540-C Description: Based on review of the center's first aid kit, one required item was not found. Evidence: The center's first aid kit did not include a working battery operated radio. Plan of Correction: We will obtain and maintain a working battery operated radio.
Standard #: 22VAC40-185-550-A Description: Based on review of the center's emergency preparedness plan, the plan does not include current information. Evidence: In reviewing the emergency preparedness plan, staff contact information is not current. Plan of Correction: We will update this plan and reflect all current staff and procedures.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation of the center, a required posting was not found. Evidence: The most recent Violation Notice dated 12/11/2018 was not posted or found. Plan of Correction: We will post today's violation notice and will continue to post the most recent one.
