Technical Assistance:
Additional space was measured in anticipation of summer camp. Please send a written statement with the capacity requested along with the check for the larger capacity. Please have custodial staff provide a label for the cleaner/disinfectant used for sanitizing tables.

Comments:
An unannounced renewal inspection was conducted with center director and center staff. There were 20 school age children present with two staff and one EEC teacher present. Three medications for two children were reviewed - please see Supplemental Summary for missing item. One staff file and three children files were reviewed - see Supplemental. The children were engaged in free time activities, ate afternoon snack, and did homework/quiet activities. Snack served today: Sun chip cheddar crackers and Capri-Sun juice. Standard #: 22VAC40-185-280-G
Description: Based on observation, the facility failed to clearly indicate the contents of hazardous substances in a substitute container. Evidence: the spray bottle with the pink solution used to clean/sanitize the tables was not labeled with the contents.

Plan of Correction: Center staff will label the spray bottle with the contents.

Standard #: 22VAC40-185-510-A
Description: Based on observation and medication review, the facility failed to have a long-term written medication authorization signed by the child's physician and parent. Evidence: Child A did not have a long term written medication authorization form for one prescription medication. The parent completed a medication authorization on 1/24/2018.

Plan of Correction: Center director will request the information from the parent.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on review of one staff file, the facility failed to have documentation of a completed sworn disclosure statement prior to the first day of employment. Evidence: Staff A (date of hire 10/5/2017) did not have documentation of a sworn disclosure statement in the file.

Plan of Correction: Center director will check with corporate to see if a sworn disclosure was completed.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of one staff file, the facility failed to have documentation of a completed central registry finding (CPS check) within 30 days of employment. Evidence: Staff A (date of hire 10/5/2017) did not have documentation of a completed CPS check within 30 days of employment.

Plan of Correction: Center director will check with corporate to check the status of the CPS check.
