Technical Assistance:
Reviewed the timeline for all required fingerprint background checks.

Comments:
An unannounced full renewal inspection was conducted today. There were 27 children in care, with a total of 11 staff members assigned to their direct supervision. Children's records, staff records, staff training records, children's medications, emergency preparedness items, the physical plant, and staff background checks were inspected today. The children were observed dismissing from the AM program, attending lunch bunch, making Valentines art, listening to stories, playing outside, and engaging in creative play activities. This inspection was completed between approximately 11:00am and 1:15pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #3 does not include documentation to indicate that a staff person has viewed the child's original proof of identification.

Plan of Correction: The birth certificate was viewed but it was not documented. I will ask the parent to bring the ID again and it will be documented.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff training documentation, it was determined that First Day Orientation Training has not been documented for all staff. Evidence: There is no documentation that First Day Orientation Training was completed for 4 of 5 staff members.

Plan of Correction: The orientation was completed but the documentation cannot be found. We will conduct orientation again, for all staff, at our next staff meeting.

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's medications, it was determined that one authorization form was not completed as required. Evidence: One child's medication authorization form does not include the parent's signature.

Plan of Correction: The parent will be asked to sign the form.

Standard #: 22VAC40-185-510-E
Description: Based on a review of children's medications, it was determined that one non-prescription medication was not labeled as required. Evidence: One child's medication was not labeled with the child's name.

Plan of Correction: It was labeled during the inspection.
