An unannoucned monitoring inspection was made on 2/9/18. There were 25 children present with four staff supervising. Five children's records were reviewed. Four staff records were reviewed on 2/15/18 and the inspection was completed on that date with the child care director. No medications at the site. Evidence: 1. There was no physical examination in the record for child #1. Child #1 first date of attendance was 1/5/18. Plan of Correction: The staff will talk with the parent to get permission to get a copy of the child's physical examination from the public school office.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that all children's records contained documentaiton of all required information. Evidence: 1. The record of child #3 was missing the following required information: the address for two of two required emergency contacts for the child, work place and work phone number for the mother and father of the child. Plan of Correction: The staff will have the parent complete the information today at pick up.
