22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:25am-3:05pm. At the time of entrance, 74 children were in care with 28 staff members present. Children were observed during snack time, infant care, engaged in group activities and outdoor play time. A fire drill was conducted during the visit. The center is clean and organized. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A Description: Based on record review, staff records did not include required information. Evidence: 1. Staff A did not have 2 references in her file and began employment on 2/24/2014. Plan of Correction: The county file has the references and I will obtain a copy of them.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center inside were not maintained in a clean, safe and operable condition. Evidence: 1. in Room 115, a blue soft back pillow had fabric that was peeling off. 2. In room 143, the wall divider had four holes in the wall the size of a dime or larger. 3. In Room 138, a orange pad that children sit on had a large hole in it. 4. In Room 105, three small chairs were missing the belts to strap the children in. Plan of Correction: In room 105, belts will be added to the chairs or we will remove the chairs. In classroom 115 and 138, the items were removed and disposed of mentioned. In room 143, we will contact maintenance today to determine if it will be repaired or removed.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: 1. In Room 115, one can of febreze air freshner was on a the sink counter top. 2. In Room 119, 2 cans of febreze air freshner were in an unlocked cabinet. 3. In Room 143, a container of clorox wipes and bleach water were in a high cabinet with the key in the lock. 4. In Room 128, there were 7 cleaning materials( febreze, fantastik, clorox wipes, soft scrub, glass cleaner and 2 fluid control solidifiers) in the bathroom in an unlocked cabinet. Plan of Correction: I locked all unlocked cabinets and moved cleaning products that needed to be moved and will review with every single classroom.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the provider failed to cover all electrical outlets accessible to children under five years old with protective covers. Evidence: 1. In Room 138, 2 electrical outlets on a surge protector were not covered. 2. In Room 128, 2 electrical outlets on a surge protector were not covered. Plan of Correction: I covered immediately.
Standard #: 22VAC40-185-510-J Description: Based on observation, the center did not keep all medication in a locked place using a safe locking method that prevents access by children. Evidence: 1. In room 138, one epi-pen medication was found in a higher cabinet that was unlocked. Plan of Correction: The epi pen will be kept in a locked container unless we receive a doctors note.
