Comments:
A Subsidy Health and Safety inspection was conducted on 2/15/18 from 2:30 pm until 4:45 pm to review supplemental health and safety requirements. The sample size consisted of five children's records, three staff's records and three sunblocks. There was no medication at the center at the time of the inspection. Children and staff were observed during snack, hand washing, bathroom break, nap, outdoor play, pick-up, transition, and behavioral guidance. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-004-F
Description: Based on observation, record review and interview, the administrator failed to post or keep in a way that is immediately accessible to staff, a current list of all children's allergies, sensitivities, and dietary restrictions. Evidence: 1. A review of child 2's record showed documentation of a sensitivity to cinnamon. The two classrooms were inspected and no list of children with food allergies, sensitivities, and dietary restrictions could be found. 2. The administrator stated they had no children with food allergies. The administrator stated she thought the requirement was only for food allergies.

Plan of Correction: A list will be made available for each staff of any child's food allergies, sensitivities, and dietary restrictions.

Standard #: VENDSUB-000-032-B
Description: Based on record review and interview, the administrator failed to maintain documentation of the staff and children completed at least one lockdown drill in 2017. Evidence: 1. The folder of emergency drills was reviewed and no documentation for a lockdown drill was found. 2. The administrator stated they did conduct a lockdown drill in 2017 and she thought she documented the drill. The administrator was unable to find the documentation.

Plan of Correction: In the future documentation will be completed for every emergency drill and will be kept for at least one year.
