22VAC40-191 Background Checks (22VAC40-191)
This unannounced monitoring inspection was conducted on 2/15/18 from 2:30 pm until 4:45 pm. At the time of the inspection 16 children were present with three staff. The sample size consisted of five children's records, three staff's records and three sunblocks. There was no medication at the center at the time of the inspection. Children and staff were observed during snack, hand washing, bathroom break, nap, outdoor play, pick-up, transition, and behavioral guidance. The violation from the previous inspection was reviewed for current compliance. One repeat violation was found. The violations found during this inspection are on the violation notice. Standard #: 22VAC40-185-130-A Description: Based on record review, the administrator failed to obtain documentation for one out of five children of immunization records as required by the State Board of Health before the child's first date of attendance.
The record for child 1 indicated the first date of attendance was 9/6/16. The immunization record in child 1's record is dated 10/21/16.
Plan of Correction: In the future children will not be allowed to start attending the center until the immunization records are received.
Standard #: 22VAC40-185-140-A Description: Based on record review, the administrator failed to obtain documentation of a physical examination for one out of five children prior to attendance or within the first 30 days of attendance. Evidence: The record for child 2 states the first date of attendance was 10/24/17. The record for child 2 does not contain documentation of a physical examination. Plan of Correction: The parents of child 2 will be advised to provide documentation of a physical examination within a week or the child will not be allowed to return until the documentation is received. In the future the administrator will ensure documentation of a physical examination is obtained by the end of 30 days of attendance or the child will not be allowed to return until the documentation is received.
Standard #: 22VAC40-185-550-E Description: Based on record review and interview, the administrator failed to maintain documentation that the center staff and children completed at least two shelter-in-place drills in 2017.
1. The folder of emergency drills was reviewed and no documentation for shelter-in-place drills was found.
2. The administrator stated they did conduct two shelter-in-place drills for 2017 and she thought she documented both of them. The administrator was unable to find the documentation.
Plan of Correction: In the future documentation will be completed for every emergency drill and will be kept on record for at least one year.
