New sworn statements and background check regulations, which can be found on the state website, were discussed with the center owner during the inspection.
An unannounced monitoring inspection was conducted from 12:50pm until 3:10pm. At the time of the tour, there were 70 children in care with 12 staff present. A sample of 7 children's records and 7 staff records were reviewed. Children were observed resting quietly during nap time, playing outside on the playground, eating snack and participating in afternoon learning activities. Restroom and handwashing procedures were observed. Medication, transportation, children's injury reports, the emergency preparedness plan, first aid and emergency supplies and documentation of emergency practice drills were reviewed. Violations in the areas of physical plant and special care provisions and emergencies are identified on the violation notice and were discussed with the center owner during the exit interview.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure that areas and equipment of the center shall be maintained in a safe condition. Evidence: In the school-aged portable there was a chair that had two torn areas with exposed foam, staples and splintered wood. Plan of Correction: The chair was removed from the classroom and disposed of during the inspection.
Standard #: 22VAC40-185-330-B Description: Based on observation the center failed to ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99. Evidence: 1. On the playground, the large composite structure had approximately 4 inches of resilient surfacing under the fall zones of the climber, ropes and slide. The singular slide had approximately 2 inches of resilient surfacing on 2 of the 4 sides of the play structure, which included the fall zone. The swings had approximately 4 inches of resilient surfacing under the fall zones. Plan of Correction: The mulch will be turned over and raked and additional mulch will be ordered as needed.
Standard #: 22VAC40-185-510-C Description: Based on medication review and interview, the center did not ensure that procedures for administering medication shall include methods to prevent use of outdated medication. Evidence: 1. The EpiPen for child #1 has an expiration date of January 2018. 2. The center owner confirmed that the Epipen was expired. Plan of Correction: The expired Epipen will be returned to the parent today at pickup.
